  • Application form, filled in properly
  • Last year's Mayor's permit or tax order of payment
  • Proof of annual gross receipts: financial statements, ITR or sales returns (2550Q or 2551Q) if BIR-registered, or a sworn declaration of gross sales or receipts if not
  • Certificate of tax exemption from local taxes or fees, if exempt
  • Board resolution or secretary's certificate (corporations and cooperatives), or an SPA or authorization letter with IDs (sole proprietors using a representative)
  • A valid fire safety inspection certificate
